Gameplay in Bloody Nightmare is structured around exploration, puzzle-solving, and combat avoidance. The player controls Dr. Vance from a third-person perspective with fixed camera angles. Movement is handled with the left analog stick or W, A, S, D keys. The right analog stick or mouse controls a flashlight, which is the primary tool for revealing hidden objects and weak points on enemies. The game features a stamina meter that depletes when sprinting, performed by holding the left shift key or pressing the left analog stick. Combat is minimal; the player can use a melee weapon (a crowbar) with the right trigger or left mouse button, but direct confrontation is discouraged due to limited health and ammunition.
